About

  • A closed rhinoplasty is a surgical procedure that alters the appearance of the nose.
  • Incisions are made inside the nose which causes less swelling and scarring than a traditional nose job.

Safety

  • Close rhinoplasties are generally considered safe.
  • They should only ever be performed by a trained, board certified plastic surgeon.

Convenience

  • The procedure itself typically takes 1 to 2 hours.
  • It requires a week of intensive healing and time away from work.

Cost

  • Closed rhinoplasties may cost anywhere from $5,000 to $25,000, depending on the complexity of the procedure and where your surgeon is based.

Efficacy

  • Close rhinoplasties are permanent procedures that can correct dorsal humps and narrow the nose tip or bridge of the nose.
  • According to a 2015 study of 2,346 online patient satisfaction reviews, the overall satisfaction rate with rhinoplasty was 83.6 percent. Additionally, females (87.6 percent) were more satisfied than males (56 percent) with the surgery results.

If you’re not totally happy with your nose, you may be looking for a way to alter its appearance without noticeable scarring. A closed rhinoplasty is a cosmetic procedure that uses incisions inside the nostrils and nose so there isn’t a lot of visible scarring on the outside.

Unlike a more traditional open rhinoplasty, closed rhinoplasty is typically used for reshaping the tip of the nose, shortening the nose, or getting rid of a bump or dorsal hump. The ideal candidate’s nose would be symmetrical with the majority of the reshaping occurring on the bump or the tip.

The cost of a closed rhinoplasty will vary depending on the surgeon and where you live. In 2019, the average cost of rhinoplasty, closed or open, was $5,409, according to the American Society of Plastic Surgeons.

Keep in mind that this cost is only for the procedure. You could have other costs related to anesthesia and the operating room.

If the closed rhinoplasty is for aesthetic reasons only, it will not be covered by insurance. You will also likely have to take off 4 to 5 days of work, which is less than with a more invasive open procedure.

In a closed rhinoplasty, also sometimes called an endonasal rhinoplasty, the surgeon will make all incisions inside the nostrils.

The nose is not fully opened and the nasal structure is not exposed. All the work is done through narrow openings in each nostril, and the surgeon has limited visibility. For this reason, it’s essential to pick a skilled surgeon and look at before and after photos of their work.

In addition to no visible scarring, the closed rhinoplasty procedure is fairly quick and does not result in a lot of swelling or a long recovery. It will likely be done under general anesthesia.

On the day of the procedure you will be given anesthesia. Some doctors prefer general anesthesia during rhinoplasty, as the procedure can result in blood in the throat, which could be potentially dangerous if a patient is unconscious.

Once you are numb, your surgeon will make incisions inside your nostrils. They’ll separate your skin from your cartilage, then begin the process of reshaping by adding cartilage or shaving or adding bone. Adding bone requires a bone graft.

When the shaping is complete, sutures or stitches will close any incisions, you’ll get a cast or a nose splint. A closed rhinoplasty procedure will typically take 1 to 2 hours.

Closed rhinoplasties target the nose, specifically the length, the shape of the tip, or the bridge.

·As with all surgeries, closed rhinoplasties carry some risk. Side effects to be aware of include:

  • breathing difficulties
  • nosebleeds
  • a numb nose
  • an asymmetrical nose
  • scars
  • allergic reaction to anesthesia
  • prolonged bruising
  • postoperative deformities resulting in the need for a second surgery

After the procedure, you’ll wear a cast or splint for approximately 5 to 7 days. This will help support your nose as it heals. Arrange in advance for someone to drive you home, as you will not be able to drive under anesthesia.

You can expect to take a few days off from work and try to sleep with your head elevated, which can help reduce swelling.

Your doctor may tell you to avoid the following:

  • strenuous activity or exercise
  • excessive chewing
  • swimming
  • drinking alcohol
  • blowing your nose
  • sleeping on your face
  • pulling clothes over your head

After a week or so, your cast or splint will be removed, revealing your new nose. You may still have some bruising and swelling around the nose and eyes, but you should begin to return to normal activities. Results are permanent unless you break or damage your nose.

Here is an illustration of what you can expect from an open vs. closed rhinoplasty.

Before a rhinoplasty, you will have a consultation with your surgeon to discuss your specific situation and desired outcome. You should make sure the doctor understands your vision and is willing to share before and after photos, and that you have reasonable expectations for what to expect.

At this appointment, make sure to disclose any supplements or medications you’re taking. Your doctor or nurse will also photograph your nose from varying angles and will help you decide if you’re a better candidate for a nonsurgical rhinoplasty, which involves fillers to reshape the nose.

In the days leading up to the procedure, avoid blood thinning medications, including ibuprofen or aspirin, and alcohol. Smoking can prolong the healing process, so it is a good idea to quit smoking before the procedure, if possible.

Arrange to have someone drive you home. If you live alone, you might as a friend to stay with you for a day or two.

Closed and open rhinoplasties both reshape and adjust the appearance of the nose. An open procedure is a more involved procedure with more swelling and scarring and a longer recovery time. It is better for severely asymmetrical or crooked noses or other complex cases involving breathing issues.

A closed rhinoplasty typically has a shorter recovery period and involves less swelling. It can be used to correct bumps, dorsal humps, or to reshape the nose bridge, bulb, or tip.

A rhinoplasty of any kind should only be performed by a board certified plastic surgeon.
